Do you support people with diabetes?
NHS Healthy Living is a free digital service available to adults living with diabetes (excluding gestational diabetes)
The service offers two tailored digital programmes;
Healthy Living for people with type 1 diabetes and
Healthy Living for people with type 2 diabetes.
For people with other forms of diabetes, please recommend the programme you feel is most appropriate.
We encourage all NHS staff to promote Healthy Living to eligible patients. Alternatively, patients can self-register at www.healthyliving.nhs.uk.
Anyone living with diabetes in England, as well as their carers, family and friends, can access:
Evidence-based diabetes education and support
Online community forums
Health tracking tools
Individual and group physical activity challenges
How do people with diabetes get access?
There are three ways to join:
Direct registration – Eligible people can join directly by selecting Sign Up at www.healthyliving.nhs.uk
HCP referral – Healthcare professionals can submit referrals on behalf of patients (see below for more details)
Via PRISM – PRISM helps GP practices identify eligible patients and can automatically update records with relevant SNOMED codes for programme uptake and completion. Further information is available below

Who provides the NHS Healthy Living service?
From September 2026 the service is delivered by University Hospitals of Leicester NHS Trust (UHL) and powered by MyDESMOND, the digital platform developed by experts at the Leicester Diabetes Centre, a partnership between UHL and the University of Leicester.
MyDESMOND is built on the internationally recognised DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed) programme, which has supported more than 800,000 people with type 2 diabetes and has been adapted for a range of long-term conditions.
For the re-launch of Healthy Living, the MyDESMOND platform has been redesigned to align with NHS branding and meets WCAG 2.2 AA accessibility standards.
UHL delivers the service in partnership with Promatica Digital Ltd, a healthcare technology company specialising in digital healthcare solutions and system integration.
Who are Promatica and how can you get PRISM integrated?
Promatica Digital Ltd support the technical delivery of Healthy Living; including integration with primary care systems and digital services, helping to create a seamless experience for both patients and healthcare professionals.

How can NHS Staff view the programmes?
NHS staff can register for staff access to explore both Healthy Living programmes, including all learning content, booster sessions and supportive trackers and resources.
Please note, access to chat forums is restricted to people living with diabetes to maintain a safe peer-support environment.
How can NHS staff refer their patients formally?
NHS staff can submit a formal HCP referral by downloading the form below, completing and returning it as an attachment to england.referralhealthyliving@nhs.net
Following receipt our system will partially complete registration and send this directly to the patient referred. Your patient will then complete any remaining registration fields before being granted access to the programme.
